Main Policies
1. I understand Tuition payments are based on a 40-week calendar with 9 equal payments (September May) and 2 weeks in August. (June may vary depending on recital date.) There is no reduction in tuition for holidays. The number of weeks in a month varies but additional tuition is not charged for months with 5 weeks. 2. I understand Tuition is due by the 1st of each month. No statement is mailed unless payment is past due. (Special circumstances will be considered individually see the office.) Payments should be made by Auto Draft, check, money order, or credit card. For security reasons, NO CASH accepted. 3. I understand that a late fee of $15 will be charged if tuition is not paid by the 10th of each month. An additional fee of $15 will be charged and classes discontinued until tuition is paid in full for accounts 60 days past due. 4. I understand if for any reason you must discontinue classes the office must receive 30 day written notice before the 1st to avoid billing for that month. You are responsible for all tuition until notification is received. There is no prorating for unused classes. Telling the teacher is not acceptable for notification. 5. I understand that classes are billed monthly, regardless of attendance. Missed classes are not pro-rated or refunded for illness and inclement weather. Holiday breaks are not included in billing cycle. Students may however attend a make-up class within 30 days of the missed class. One make-up class is permitted for illness or family emergency per semester. If you are temporarily out for extended illness or travel, notify the office so billing does not continue during that time. 6. I understand that ALL fees (registration fee, tuition, recital, costumes, etc) paid to KDT, are NON-REFUNDABLE. Free trial classes are available to try a different class or teacher. 7. I understand KDT has a required dress code to attend classes. 8. I understand that all class placement is subject to teacher approval and it may take several weeks to complete teacher evaluation. KDT instructors have the authority to change a students class placement for irregular attendance. KDT reserves the right to dismiss any student from class who is continuously disruptive. 9. I will bring my concerns/complaints about KDT to Mary Lee, not to other parents or students. Non-compliance of this policy will result in dismissal from the school.

Indemnity Waiver
1. I understand that physical activity in dance class involves a certain amount of risk. I release Kennedy Dance Theatre, Dance Tree Inc.and their executives, agents, instructors, and employees from any claims, actions, causes of actions, damages, costs, liabilities, expenses or judgements, including attorneys fees and court costs, arising from the below named student's and/or signator's participation in any Kennedy Dance Theatre or Dance Tree Inc.activity. 2. I give permission for the below named student to be photographed and/or videotaped during the course of any Kennedy Dance Theatre or Dance Tree Inc.activity with the understanding that the photograph(s) and/or video tape(s) may be used by Kennedy Dance Theatre or Dance Tree Inc. 3. The below named student and/or signator shall be held responsible for any injury to themselves, their assigns, heirs, and agents for any injury or loss or injury to property while at any Kennedy Dance Theatre or Dance Tree Inc. activity. 4. The below named student and/or signator agrees to assume any and all liability of any kind whatsoever from the use of or presence on the Kennedy Dance Theatre studio or while at any Kennedy Dance Theatre or Dance Tree Inc. activity and to indemnify, protect, defend, and hold harmless Kennedy Dance Theatre, Dance Tree Inc.,and their executives, agents, instructors, and employees from and against any and all liability, claims, demands, suits, actions, damages, losses, and expenses, including any and all attorneys' fees necessary in defense of any such action. It is the intention of the user that such indemnity shall apply whether or not the liability, claims, demands,suits, action, damages, losses or expenses arise from the negligence or omissions of Kennedy Dance Theatre, Dance Tree Inc.,their executives, agents, instructors,or employees. 5.The below named student and/or signator agree(s) that Kennedy Dance Theatre, Dance Tree Inc.,their executives, agents, instructors,and employees shall not in any way be liable to the below named student and/or signator, their assigns, heirs or agents for damages of any kind whatsoever which may accrue to themselves, their assigns, heirs or agents by reason of their participation or attendance any Kennedy Dance Theatre or Dance Tree Inc. activity, whether or not the damage may arise by any act or omission of the Kennedy Dance Theatre, Dance Tree Inc., their executives, agents, instructors, or employees. 6.By the below signature, I execute this waiver, release, and indemnity agreement to Kennedy Dance Theatre and Dance Tree Inc. and permit the below named student to participate in the Kennedy Dance Theatre/Dance Tree Inc.program.
